2015-2016 Enrollment Packets
USD 273 enrollment will look slightly different this year. In order to save money on postage, we will not be mailing enrollment packets to each family home. We have posted all enrollment documents here to print and complete at your convenience. If you do not have access to a printer, contact the school and a packet will be mailed. You can also pick up packets during normal business hours prior to enrollment. Thank you for your cooperation.

Enrollment Dates:
Beloit Elementary: Thursday, August 6th, 8:00-4:00
Friday, August 7th, 8:00-12:00
Beloit Jr./Sr. High: 8th & 9th Grades: Thursday, August 6th, 7:30-12:30
10th - 12th Grades: Friday, August 7th, 7:30-12:00
7th Grade: Tuesday, August 11th. 7:00 p.m.

Beloit Elementary - Returning Student & Pre-enrolled Preschool - print one per student
Beloit Elementary - New Student to the District - print one per student
Beloit Elementary - New Preschool, Not Pre-Enrolled - print one per student
Beloit Jr./Sr. High enrollment documents
Returning Jr-Sr High students - including pre-enrolled 7th graders - print one per student
New Jr-Sr High student - print one per student
Free & Reduced Food & Textbook application:
Free & Reduced Application - print one per family. Complete and date application after July 24th. All forms dated before July 24 will have to be redone.

Insurance enrollment forms & waivors declining insurance are not available yet. Check back before enrollment to print this document. Or the form will be available at the building on enrollment days.
